I have permission from the AAFA to post a survey passed around at the Product Safety Council meeting in Washington DC last week. This form (pdf) is to be used to quantify the costs of a recall for those who have been through one. Obviously, we are trying to get a dollar figure on costs of actual product recalls to present to the CPSC before National Bankruptcy Day. For your convenience, I OCR’ed the document into a text document so you can put it in a word processing program and fill out on screen. If you have not been through a recall, I will be modifying this form to create a survey that anyone can use to estimate their potential losses. Feel free to re-post this entry as needed.
