Using the invaluable information from this website, I created this &lt;a href="http://www.daisyjanie.com/2006%20Fall%20Winter%20Catalog/linesheet_fall2006_orderform__JAN.pdf" rel="nofollow"&gt;ORDER FORM&lt;/a&gt; and had it printed &lt;b&gt;&lt;i&gt;while I waited&lt;/i&gt;&lt;/b&gt; at OfficeMax (not Staples or Office Depot*) as a 3-part, tri-color, carbonless form, for 34 cents each!! Print as many or as few as you like, still 34 cents. No kidding. That copy guy received a stellar recommendation from me for making my day!

I've also had good luck with this site, but the minimums might be too much for some: &lt;a href="http://www.ncrforms.com/home.php" rel="nofollow"&gt;NCR Forms&lt;/a&gt;.

*These big box office suppliers either had to send it out, with a large minimum and a surcharge for a rush, or didn't even know what I was talking about. Yikes.
