Using the invaluable information from this website, I created this ORDER FORM and had it printed while I waited at OfficeMax (not Staples or Office Depot*) as a 3-part, tri-color, carbonless form, for 34 cents each!! Print as many or as few as you like, still 34 cents. No kidding. That copy guy received a stellar recommendation from me for making my day!

I’ve also had good luck with this site, but the minimums might be too much for some: NCR Forms.

*These big box office suppliers either had to send it out, with a large minimum and a surcharge for a rush, or didn’t even know what I was talking about. Yikes.
